Change of Name Certificates & Documents

How it works

The Government keeps a record of all registered legal changes of name that take place in Alberta and can only issue documents for these.

There are 2 kinds of legal change of name documents you can order:

  • Legal Change of Name Certificate
  • Legal Change of Name record search

Legal Change of Name Certificate

This certificate includes:

  • full name of the person before the legal change of name was processed
  • full name of the person after the legal change of name was processed
  • registration number
  • registration date
  • date certificate is issued

All certificates are certified extracts of the original legal change of name registration.

The record search includes:

  • full name of the person before the legal change of name was processed

What’s important to know

Altering and/or laminating Vital Statistics documents makes them invalid.
