How to apply

You can order a legal change of name document from within or outside of Alberta.

What’s important to know

There is a $20 government fee for ordering legal change of name documents, as well as registry agent or Registry Connect service fees. Fees vary among registry agents.

Documents are usually mailed to you 2-3 days after a registry agent or registry connect processes the application. If information is missing or there are any discrepancies in information, an application may be delayed. Documents are sent to you by regular mail. For special arrangements or rush service options, speak with a registry agent or Registry Connect.

Ordering a legal change of name document within Alberta

  1. Complete the form

    Applications are also available at registry agent offices.

  2. Drop off the application

    Take the completed application to a registry agent, along with an accepted form of ID.

Ordering a legal change of name document from outside Alberta

  1. Complete the form

    Registry Connect Application for Certificate / Documents (PDF, 8 pages)

    Don’t complete the Statutory Declaration for Proof of Identity Portion of the form at this time.

  2. Go to a Notary Public or Commissioner for Oaths

    Take the completed application and your accepted ID to a Notary Public or Commissioner for Oaths to complete the Statutory Declaration for Proof of Identity portion of the forms.

  3. Mail your application

    Mail or courier the original completed application and Statutory Declaration for Proof of Identity to Registry Connect.
