How to apply

Request a correction to an Alberta record.

You can request a correction by either visiting a registry agent or by mailing in a request.

What’s important to know

It usually takes 8 to 10 weeks to complete an amendment.

  1. Complete the form

    Request to amend an Alberta Vital Statistics Registration (PDF, 1 page)

  2. Send the application

Send the completed application to Vital Statistics.

Through a registry agent

  1. Complete the form

    Request to amend an Alberta Vital Statistics Registration (PDF, 1 page)

  2. Drop off the application

    • take the completed application to a registry agent office
    • the registry agent may collect the amendment fee, and may also charge a service fee

The registry agent will forward your amendment request to Vital Statistics.

Vital Statistics will contact you regarding the required documents, evidence and applicable fees.
